1. In a small (1-quart) slow-cooker, add condensed milk, caramel topping, and chocolate; stir to combine.
2. Cover and cook on LOW for 2 hours.
3. Stir until mixture is smooth.
4. Serve immediately.
5. I like to serve this with different fruits, especially strawberries.
6. If mixture thickens, stir in a little warm milk.
